Here's my .02.  I found a web page for a guy that made his own.  He gives
Harley part #s and complete instructions on how he did it.

This is a do it yourselfers project.  The non-mechanically inclined need not
apply.  :-)

I used forward controls from a Harley on a Honda Chopper.  I had to
fabricate
some brackets to match the mounting holes on the Honda to the holes on the
controls.  It took a bit of fiddling but wasn't all that hard.  I used
controls off a 70 HD that operated the brake pedal with a rod.  Newer units
have the master cylinder right on the controls and will need new brake
lines.
 I found mine at a swap meet for about $50.00.
